聲樂家罹甲狀腺癌 碘131恢復好嗓音

台中一位女聲樂家四年前發現甲狀腺長腫瘤，卻擔心手術會影響聲音及頸部留疤，遲遲不願就醫，直到聲音越來越沙啞，不得不就醫尋求協助，所幸透過「經口腔內視鏡甲狀腺切除術」切除病灶，再將切片送驗，證實為惡性甲狀腺乳突狀細胞癌，後續安排「碘-131」治療順利出院，並且重新站上舞台。
亞大醫院外科副院長義煇表示，正值中年的患者身兼聲樂家和劇團負責人，對自我要求極高，四年前發現左側甲狀腺有一顆大腫瘤，當時醫師懷疑為惡性，力勸開刀治療，但因患者擔心術後影響表演生涯，決定與之和平共處；直到最近聲音沙啞、脖子日益粗大，赴醫先透過超音波檢查，證實右側甲狀腺有一顆4.5公分大的腫瘤，進一步穿刺檢查也發現有非典型細胞，高度懷疑為惡性腫瘤。
但患者仍然擔心傳統手術後會在體表留下疤痕，溝通後，決定接受「經口腔內視鏡甲狀腺切除術」，術中也採用「使用喉返神經偵測系統」，減少傷及喉返神經的機率，使術後聲帶麻痺的可能性降到最低，也讓患者能保持美妙歌聲。
核子醫學科主治醫師陳育仁也指出，依國健局公布的癌症資料顯示，甲狀腺癌高居女性癌症第五名，超越過去常見的子宮頸癌、胃癌、卵巢癌等，並有逐年增加的趨勢，所幸大部份甲狀腺癌的分化良好，10年存活率可超過90%，且早期治療效果相當好，越早發現存活率越高。
陳育仁強調，外科手術是分化良好的甲狀腺癌最主要的治療方式，術後會再以碘-131清除殘餘的正常甲狀腺組織，其功能有二，首先當正常甲狀腺組織手術清乾淨後，患者將不會產生甲狀腺球蛋白，故未來可藉由追蹤甲狀腺球蛋白指數，以利早期發現癌症復發；再者若甲狀腺癌復發，此時正常的甲狀腺已經被清除，所以不會與癌細胞競食碘-131，碘-131才能有效被甲狀腺癌細胞吸收。
陳育仁建議，由於中部地區醫院設置碘-131病房的數量有限，不少患者往往得等候多時，因而影響就醫意願，所幸目前院內設有兩間碘-131隔離病房，可避免病人服完高劑量的放射碘藥物，體內會有輻射量產生，直接影響他人，可先住進病房隔離一天始能離院，病房內部寬敞明亮，病人與醫護人員可透過螢幕及對講機溝通，提供舒適與安全的治療環境，讓患者安心康復。
